Contributor Release Form-

Before i started with the production of my video i asked the actors that were involved in filming to sign a contributor release form to ensure that i had their permission to film them on camera. These forms also prevent anyone who doesn't want to be filmed or doesn't want to be in the documentary from being included in the final edit. The only actor in my documentary was my friend James who i asked to sign a form if he was happy to be in the final product and i got his permission to have his in the documentary as he signed a contributor release form.

Location Release 

To ensure i knew exactly where i was filming my video i completed my own version of the location scouting forms, which can be seen above on the second page. In these forms in it will include the address, times of filming, dates of filming, etc. These forms are also important in the filming industry to make sure they have permission to film there and to know exactly where they are filming on certain days and how long they will be at that specific location for. These forms will also be given to the property manager to ensure they are also aware of the filming times and dates.

Personal list 

Before i started filming my documentary i also decided to create my own personal list so i could see the different cast members that i would be having in my documentary. However, in my documentary i only have one actor throughout the whole product. I still found it useful to make the list to show a visual representation of the actors role. My personal list consists of the actors full name and their role in the documentary.

Risk assessment

Before i started with the production of my video i completed a risk assessment form to ensure that the location that i would be filming in were safe for both the cast and the crew, this was to make sure that the filming process would be as safe as possible for everyone involved. On the last slide is my own version of a risk assessment form for: My personal address. Forms were completed fully to make sure the location was safe to film at.
